Medical Records, Confidentiality, Prescriptions, and Controlled Substances

Office Hours

  • Monday: 10 am – 6 pm
  • Tuesday: 10 am – 6 pm
  • Wednesday: 10 am – 6 pm
  • Thursday: 10 am – 6 pm
  • Friday: 10 am – 6 pm
  • Saturday: 12 pm – 4 pm by appointments only.
  • Sunday: closed
  • Office hours are by appointment. Walk-ins are welcome and depending on availability, patients can/may be seen same day appointment. Please schedule an appointment at 952-395-5222
  • Initial evaluations are 45 minutes to 1 hr. 
    Follow-ups: 15 minutes to 30 minutes, please schedule your time accordingly.
  • If you need to reschedule appointments, call 24 hours before your appointment. If you fail to contact us in at least 24 hrs, you may be charged for not cancelling in advance of your appointment.
  • No-shows of 2 or more appointments or a history of last-minute cancellations: your provider may not be able to continue treatment with you, and you may be terminated from our clinic.

Session Fees

We take most of insurances in Minnesota, if you are not covered by insurance, full fees are expected before your sessions. If you need to make payment arrangements, please contact our staff at front desk at 952-395-5222. You are responsible for all your fees and prompt payment ensures continued care.

Medical Records

Medical records can be obtained by filling patient records, if records need to be sent to another office, please provide details such as fax numbers, address, emails, phone numbers. Please make sure you complete release of information prior to requesting records.

Controlled Substances Policies

  • If there is evidence of abuse, misuse or an inability of a patient to control their use, no further prescriptions for controlled substances will be given. 
  • Repeated requests for early refills will result in a patient being discharged from the practice.
  • Our providers DO NOT prescribe any narcotic pain medications.
  • If you are taking narcotic pain medications, abusing drugs and alcohol our prescribers may not prescribe any controlled substances.
  • Taking controlled substances other than how it is prescribed, is grounds for termination of care.
  • Our prescribers may randomly check toxicology screens before and after prescription of controlled substances.

Telehealth Privacy Policy:
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Prescription Refills

  • Medications will be managed and refilled during your appointments- please plan ahead!
  • During your visits we can confirm medication you are taking, dosages, discuss with you potential side effects and send refills on time.
  • It is your responsibility as the patient to notify your physician if you need a refill at your scheduled appointment.
  • If you are overdue for a follow-up, or missed your last scheduled follow-up, we will require an appointment prior to refilling medications.
  • Failure to show or frequently cancelling your appointments at which your medications would be refilled, without rescheduling, is considered non-compliance with our office policies and you may be dismissed from the practice.
  • Please note that no refills will be called in after-hours or on the weekends or on holidays.
  • You must attend all scheduled appointments with your Doctor if you wish to continue obtaining prescriptions for your current medications.
  • If you have failed to follow up appropriately, it is at the physician’s discretion how to handle your refill requests.

Do You Accept Insurance?

Our providers accept most major insurances including HealthPartners, U Care, Blue cross/blue shield, Medicare, Tricare, Medicaid, medical assistant and many more. You may call our office at 952-395-5222 about coverage costs and clinic charges. If you are unable to use insurance to cover charges, you may pay upfront at a discounted rate at the time of your appointment.

Where do I park?

There is plenty of parking at our office site, and we are handicap accessible.

What is your cancellation policy?

If you need to cancel or reschedule your appointment, please do so before 12pm the previous business day to ensure that you do not accrue any late fees.

These charges are done on a case by case basis (we understand things come up sometimes that are out of your control).

How does a psychiatrist differ from a counselor or a psychologist?

Psychiatrists are mental health professionals who first trained to be a medical doctor, then completed at least 4 years of rigorous specialized training. In turn, they can prescribe medications, and provide counseling. We also work closely with psychologists, therapists, and counselors in order to ensure your care is optimized.
